Attention Employees: Annual Benefit Open Enrollment is May 12 - 23.

There is no paperwork required for employees who elect not to change their medical or dental benefits at this time.

However, those who are enrolling in a flexible spending account (medical and/or dependent care) are required to complete this FSA Enrollment Form for plan year 7/1/2008-6/30/2009.
